This information page is designed to provide everything you need to know if you’re a new or existing patient, coming for a procedure or you just want to discuss the condition of a loved one. Important Guidelines for First Appointment 

Please bring:

  • A photo ID
  • Your insurance card(s)
  • Physician referral forms if required by insurance
  • A list of current prescription or over-the-counter medications you are taking, including dose and frequency
  • Pertinent information about your medical and surgical history
  • Any pertinent records, films or test results you may have
  • Any patient forms you might have filled out in advance (see below)

Appointments and Referrals
All appointments related to WORKMAN’S COMPENSATION or PERSONAL INJURY must have PRIOR APPROVAL before an office visit can be scheduled.

If you have an HMO or POS plan, please be sure to bring a referral or call our office to see if your Primary Care Physician (PCP) has sent it to our office. This will expedite your appointment and prevent your insurance from denying your claim. If you are not sure whether you need a referral to see a specialist, please contact your insurance company. Patients with appointments whose referral has not been received within 24 hours of the appointment will have to be rescheduled.

Payment Information
Our office participates with many different insurance plans including Medicare, Health First Health Plans, BlueCross BlueShield and United Healthcare. Please contact the office for a complete list or you may contact your insurance company to verify participation.

Patients are required to know the benefits and eligibility requirements of their individual plans. Please direct any specific questions about insurance verification and coverage to Patient Financial Services prior to your appointment. They are available at 321-725-4500 and will be happy to assist you.

Co-payments and deductibles will be collected upon check-in. This will help facilitate your discharge from the office. As a courtesy we will file your insurance claims for you provided that your coverage is in effect at the time of service. Self-Pay patients (no insurance coverage) will be expected to pay for the office visit in full at the time of service, unless previous arrangements have been made with the business office.

Arriving to Your Appointment
We realize your time is valuable and strive to make every effort possible to see our patients on schedule. Please assist us by arriving to your appointment 15 minutes prior to your scheduled appointment in order to complete the registration and check in process. If you are unable to keep your office visit or testing appointment, please contact us at least 24 hours prior to your appointment. A fee may be charged if proper notification is not received.
